5.1 Entomology for PacELF Anti-vector measures against the mosquitoes that transmit lymphatic filariasis in the Pacific have proven their efficacy in the past, particularly where the parasite is transmitted by Anopheles. On this basis, one might predict success in eliminating filariasis from Vanuatu before the other Pacific island countries as the same mosquito, An farauti, transmit both malaria and filariasis and an effective anti-mosquito measure for malaria control insecticide impregnated bednets ; is already in place. The efficacy of MDA is enhanced in anopheline transmission areas, as anophelines do not exhibit limitation. The lack of proven interventions against the daytime outdoor biting Aedes that transmit filariasis in Polynesia, New Caledonia and Fiji precluded the incorporation of a mosquito control strategy in the present PacELF program. Aedes polynesiensis presents a particular challenge to control programs based solely on MDA as its efficacy as a filariasis vector increases with diminishing mf densities. Like the earlier DEC based MDA programs in Samoa and French Polynesia, the DEC albendazole MDA is having a dramatic impact on filariasis prevalence as measured by the ICT test. The challenge in achieving filariasis elimination will be to convince individuals to continue their participation in the MDA program, particularly the asymptomatic mf infected individuals who are more likely to suffer adverse reactions following DEC albendazole treatment. If compliance levels to this drug combination fall or if DEC with albendazole fails to kill adult W. bancrofti and stop transmission, then the present strategy of reliance on MDA alone will need to be re-examined. In its place will need to be a multifaceted program integrating effective vector control to limit transmission with measures against the filarial worm to eliminate human infections.

This disorder is mainly due to a sharp breakdown of the oxidation reactions in the mitochondria, especially the redox-enzyme system, resulting in acute malfunction of energy production for the specific cytophysiological operation of the neuron. The disturbance of the oxidation reactions in these cells, can be provoked by direct damage to the enzyme systems, providing for the intracellular breathing, by impeding the oxygen supply in the neuron and by limiting the contribution of the monosaccharides the main substance subject to oxidation in the neuron cell Table 6 ; . The first mechanism lies is the basis of most exotoxic diseases, leading to consciousness disorders. This type of hypoxia represents primary intracellular hypoxia. The second mechanism leads in an indirect way to hypoxia of the neuron, by limiting oxygen and monosaccharides supply to the cerebrum. It is observed in a number of different diseases of the organism with, for instance, albendazole oral suspension.
712 DYNAMICS OF INTESTINAL HELMINTHIASIS AMONG PRIMARY SCHOOL CHILDREN IN AN ENDEMIC FOCUS FOR SCHISTOSOMA MANSONI, NILE DELTA, EGYPT. El-Morshedy HA, Farghaly AG, Barakat RM. School age children are particularly vulnerable to acquire intestinal parasites; the impacts of infections on their nutrition, immune status, growth, productivity and cognitive functions are well documented. In the present study, school age children were followed longitudinally to study the dynamics of transmission of intestinal helminthes in an endemic focus for Schistosoma mansoni where praziquantel mass chemotherapy was applied annually within the context of the National Schistosomiasis Control Program in Egypt NSCP ; . The study encompassed 15 primary schools in which about 7000 children were enrolled. The surrogate of diagnosis was two Kato slides each of 41.7 mg. Diagnosis was repeated annually for 6 successive follow ups, 1996 - 2002. Results revealed that, at base line, 32.3 % of children were excreting one parasite, while 48.9 % harboured multiple infections. The parasites detected were; A. lumbricoides lumbricoides 67.5% ; , Trichiuris trichura 52.4% ; , Hymenolepis nana 9.1% ; and Fasciola spp 3.2 % ; . There was a significant positive correlation between A. lumbricoides infection and both Trichiuris and Fasciola spp infections. S. mansoni positive children were at higher risk for both ascarisis and trichiuriasis. Prevalence of Fasciola spp was constant all through the study. Hymenolepiasis showed a significant reduction in prevalence after the first annual follow up. As for A. lumbricoides and Trichiuris, the indices of infections remained stable from 1996 through 1999, however a single application of albendazole in 1999 has led to a considerable decline in their prevalence. This situation calls for considering combined application of mass treatment of PZQ and albendazole to control intestinal helminthiasis, in addition to other measures stressing on personal and food hygiene.

Particle Size & Swelling Index of microspheres The particle size of the microspheres was determined by using optical microscopy method.22 Approximately 100 microspheres were counted for particle size using a calibrated optical microscope Labomed CX RIII, India ; . For estimating the swelling index, the microspheres ~100 ; were suspended in 5 ml simulated gastric fluid USP pH 1.2 ; .23 The particle size was monitored by microscopy technique every 1 h using an optical microscope Labomed CX RIII, India ; . The increase in particle size of the microspheres was noted for upto 8 h and the swelling index was calculated as per method described by Ibrahim et al. 24 The swelling index for microspheres of batches B1 to B9 reported in Table 2. In vitro wash-off test for microspheres The mucoadhesive properties of the microspheres was evaluated by in vitro wash-off test as reported by Lehr et al.25 A 1x1 cm piece of rat stomach mucosa was tied onto a glass slide "3 * 1" ; using thread. Microspheres were spread ~50 ; onto the wet rinsed tissue specimen and the prepared slide was hung onto one of the groves of a USP tablet disintegrating test apparatus. The disintegrating test apparatus was operated whereby the tissue specimen was given regular up and down movements in the beaker of the disintegration apparatus, which contained the simulated gastric fluid USP pH 1.2 ; . At the end of 30 min, 1 h and at hourly intervals upto 10 h, the number of microspheres still adhering onto the tissue were counted. The results of in vitro wash-off test of batches B1 to B9 are shown in Table 1 and 2 respectively. S Lymphatic Filariasis LF ; in 2003, through GSK's partnership with the WHO in the Global Programme to Eliminate Lymphatic Filariasis, we donated 94 million treatments of albendazole to 34 countries to prevent transmission of one of the world's most disabling tropical diseases. LF currently affects 120 million people and threatens over one billion in 80 countries. In one day in July 2003, ten million people in Sri Lanka received albendazole tablets donated by GSK through the mobilisation and involvement of 50, 000 volunteers. Now in its fifth year of collaboration, the company has provided free of charge a total of 240 million albendazole treatments. Over the anticipated 20 year life of the programme, our donations to this important endeavour will build to an estimated six billion tablets, valued at $1 billion. In addition GSK provides financial grants and staff expertise to support the global effort. s Malaria The GSK African Malaria Partnership disbursed the first grants in a $1.5 million three year initiative to combat a disease that kills over one million people every year. In Sudan, Uganda and five west African countries nearly two million people will be reached through behavioural development programmes that will benefit whole communities and particularly target those most vulnerable to malaria young children and pregnant women. In October, when severe flooding in Sudan led to a malaria outbreak, GSK provided an emergency assistance grant of $25, 000 to our partner there, Plan International, to assist with relief operations.
