Adverse effects of LABA monotherapy in patients with asthma are likely due to myriad factors. LABAs may allow for asymptomatic airway inflammation by preventing bronchospasm in asthmatic patients. Suppressing symptoms could hinder recognition of worsening asthma control by patients and make them less likely to seek further treatment. LABAs may also prevent SABAs from working properly at the 2-receptor by altering the function and population of receptors in the airway. Finally, LABAs may have different dose responses with the various 2-receptor polymorphisms expressed. Specific receptor polymorphisms may predispose certain patients to diminished pharmacologic effect of the LABAs and may facil566 JAOA Vol 106 No 9 September 2006, for example, carvedilol prospective randomized cumulative survival.

There is currently no commonly acceptable definition of the term "medical accident." The Ministry of Health, Labour and Welfare of Japan defined this term in the "Guidelines for Medical Safety in National Hospitals and Sanatoriums" in 2003, stating that: "Medical accidents contain all kinds of events causing injury or death of a person that may occur at any point during the entire process of health care provision, including cases such as where the victim is a health care worker and a person who has a fall in a corridor of a medical institution." However, this description does not provide a clear definition of medical accidents, as it only means "all kinds of events causing injury, because carvedilol sr.

The age-sex distribution of respondents was similar to the distribution for all BEACH encounters, with the majority 57.8% ; of patients being female. The prevalence of chronic heart failure CHF ; in this general practice patient sample was 4.1% 95% CI: 2.85.3 ; . In male patients, 4.7% were diagnosed with CHF compared with 3.6% of female patients. Patients aged 75 + had the highest age-specific rates, with 19.8% diagnosed with CHF. CHF was classified as mild in 50.0% of these 116 patients, moderate in 28.5% and severe in 21.6%. Medication data were provided for 112 of the CHF patients. Medications most commonly used for the management of CHF were diuretics 33.6% of all listed medications ; , followed by anti-hypertensives 31.0% ; , beta-blockers 13.7% ; and cardiac glycosides 8.0% ; . The diuretic commonly used was frusemide median reported prescribed daily dose PDD ; 40 mg ; . The most common anti-hypertensive medications were perindopril median PDD 4 mg ; , ramipril median PDD 5 mg ; and irbesartan median PDD 300 mg ; , and the betablocker commonly used was acrvedilol median PDD 25 mg ; . Digoxin had a median PDD of 0.125 mg. Sixteen 66.7% ; of the 24 patients with severe CHF were on three or more CHF medications, while only 8 4.3% ; of the 56 patients with mild CHF were on three or more CHF medications. Pharmacological treatment was initiated by a GP 47.1% of CHF medications ; or by a specialist 52.9% ; at similar rates. GPs considered the factors of `symptom management' and `quality of life' significantly more important than `increased survival' as an objective of management. The majority 80.2% ; of patients diagnosed with CHF had been referred to a cardiac specialist; 38.7% were initially referred more than three years ago; 21.7% were referred between one to three years ago; and 19.8% were referred during the previous 12 months. Multiple investigations could be reported as being used in diagnosing CHF. Chest X-ray was used to diagnose CHF in 72.3% of cases, echocardiography was used in 63.4% of cases and ECG in 58.9% of cases. GPs ordered 64.9% of chest X-rays, 13.4% of echocardiography and 59.3% of ECGs, with cardiac specialists ordering the rest. Correspondence to: Graeme Miller, AGPSCC. Enteric nervous system of the guinea-pig gastro-intestinal tract: Partner 1 ; Previous studies have shown that nerve cell bodies with the shape of Dogiel type II neurons in both myenteric and submucous ganglia in the guinea-pig ileum, express NeuN, a neuronal nuclei antibody, immunoreactivity IR ; , suggesting that NeuN is a selective marker for IPANs. NeuN was initially described as a result of attempts to identify novel proteins involved in the regulation of neuronal phenotype. The appearance of NeuN is coincident with neuronal differentiation and the neuron's exit from the cell cycle. NeuN IR has been detected in several neuronal populations. The present study aimed to reveal if NeuN is a selective marker of IPANs in the guinea-pig gastrointestinal tract as previously hypothesised. NeuN IR was found in the enteric nervous system with the exception of the esophagus. Two types of NeuN-expressing neurons were observed: neurons with immunostained nuclei and cytoplasm NeuNNC ; and neurons only expressing IR in their nuclei NeuNN ; . NeuNNimmunoreactive neurons were only found in the myenteric plexus of the gastric corpus and of the proximal colon. In the gastric corpus, none of the NeuNN-expressing neurons, of which 553% co-expressed calbindin, had a typical Dogiel type I or II morphology. The NeuNNpositive neurons of the colon, which did not express calbindin, did not resemble a clear Dogiel type II morphology either, but were small-sized neurons. NeuNNC-immunoreactive neurons were observed in both the small and large intestine. These neurons were smoothcontoured and bigger-sized, resembling a clear Dogiel type II morphology. Some of these neurons co-expressed calbindin submucous plexus: 31% in duodenum, 192% in ileum, 612% in colon; myenteric plexus: 691% in duodenum, 671% in ileum, 100% in colon ; . The present data clearly revealed the presence of two types of Dogiel type II neurons, exhibiting NeuNNC + calbindin + or NeuNNC + calbindin- IR, in the small and large intestine. Both types had a region-specific distribution in the intestine. Assuming that all IPANs in the guinea-pig gastrointestinal tract exhibit a Dogiel type II morphology, we conclude that the cytoplasmic expression of NeuN is an exclusive feature of IPANs. Similar to observations made in the rat esophagus, this study revealed that also the myenteric plexus of the guinea-pig esophagus harbours calbindin-immunoreactive neurons. In the cervical and thoracic regions of the esophagus, approximately 5% of all intrinsic neurons expressed calbindin, while no calbindin-positive neurons were present in the abdominal region. At present, the physiological role of these calbindin-immunoreactive esophageal neurons remains unknown. Effects of interleukines on the enteric nervous system in the guinea-pig: Partner 1 in collaboration with the Department of Medical Physiology and Surgery, UMC Utrecht, The Netherlands ; In previous experiments, we demonstrated i ; that myenteric interstitial cells of Cajal ICCs ; express the interleukin-1 IL-1 ; receptor, and ii ; that IL-1 has a marked, direct excitatory effect on myenteric ICCs in the guinea-pig colon, indicating that these ICCs may be involved in the neuro-immune regulation of the gastrointestinal motility. To complete the experiments, we used the IL-1 receptor antagonist rHuIL-1ra in a conventional myenteric plexus 2 + 2 preparation loaded with Fluo-4-AM. Measuring the cytosolic Ca concentration [Ca ]i ; by confocal laser scanning microscopy, we shown that this antagonist completely blocked the!
