It is often effective to begin patients on antidepressants before beginning therapy, particularly if the patient has experienced psychiatric problems in the past. Patients with a history of neuropsychiatric disorders have an increased risk of experiencing symptoms during treatment. It is advised and often necessary to involve the help of a psychiatrist or therapist in the management of patients with pre-existing psychiatric illnesses. One third of patients experience alopecia hair loss ; with interferon treatment. Reassurance that hair will return upon cessation of treatment is often helpful, but thinning hair may be distressing to many throughout the course of the treatment. If weight loss persists, a consult with a dietician can be useful. Weight loss is often due to nausea and anorexia, so controlling these symptoms is essential. Many patients experience taste alteration, which may cause anorexia. Red meat may taste bitter. Chicken, fish, beans or peanut butter may be more palatable sources of protein. Marinating meats to change the flavor can help as can serving foods cold or at room temperature. We had conductive floors, wore conductive shoes, and the OR staff was required to wear cotton underwear. One EENT physician was insistent that I use open drop ether anesthesia for his tonsillectomies and then he proceeded to use an ancient gooseneck lamp and head mirror for lighting! He relented only after being shown the report of 63 operating room explosions with 58 deaths occurring in 1959. Non-flammable anesthetic agents had completely replaced explosive anesthetics in our operating rooms by 1967. EKG monitoring was introduced as a standard in the OR that same year. Pulse oximetry to record the heart rate ; was not introduced until the 1980s. General anesthesia for vaginal delivery was common when I arrived but had largely disappeared by 1965-66. Some form of regional anesthesia e.g. saddle block, caudal or pudendal nerve block replaced it. The normal caseload for anesthesiologists was 1, 200 to 1, 300 cases per year. I shared night call with the CRNA, always being available for help if needed, for two and a half years. In 1965 Dr. John Wright joined the anesthesia staff and we shared night call giving me a bit more free time. We had the opportunity to work with some really talented and gifted surgeons. Over 40 years, great strides in medicine have placed even greater responsibilities on anesthesia. With these changes, great advances have been made in monitoring anesthetized and recovering patients. Patient safety has been greatly improved with the addition of improved inhalational agents, muscle relaxants, local anesthetics and pre-operative antibiotics. Pain management has been enhanced through the use of nerve blocks, epidurals and intrathecal injected into the spinal canal ; narcotics, and patient self-administered narcotic pumps. I credited the development of the post-operative recovery room with lessening the impact of surgical and anesthetic morbidity for patients. If you are experiencing persistent daytime sleepiness, talk with your doctor about taking the adjunct medications discussed on page 7. Nausea may occur when taking an opioid drug for the first time. Generally, the nausea goes away within a short period of time. Anti-nausea drugs, such as prochlorperazine Vompazine and others ; , a scopolamine patch Transderm Scop ; , or metoclopramide Reglan and others ; may be helpful for the first several weeks after starting an opioid. It's important to remember that nausea may also be a side effect of constipation, so it's important to maintain regular bowel movements. If nausea persists, talk to your doctor. Itching of the face, scalp, head and neck is a very common side of effect of opioids. The itchy feeling is not a rash or an allergy, and it will generally disappear within a short time. But if itching persists for more than a few days, you may need to switch to a different opioid.

Treatment of Acute Porphyria Supportive Treatment. Most patients with acute hepatic porphyria are ill and need to be admitted to a hospital. In milder attacks, the patient may be treated on an outpatient basis. The key to supportive treatment is to maintain nutrition. This is best achieved by a high-protein, high-carbohydrate diet of at least 1000 to 2000 calories per day. If nausea and vomiting precludes oral feeding, calories have to be given IV with dextrose saline infusions and intermittent boluses of concentrated 50% dextrose, given piggyback to maintain fluid restriction. This nutritional treatment, also known as the glucose effect, is thought to suppress hepatic heme production. Fluid and electrolyte balance need to be meticulously restored and maintained. If pain is prominent, it can be treated safely with meperidine hydrochloride or morphine. Nausea and vomiting can be controlled with prochlorperazine Compazone ; or chlorpromazine Thorazine ; given orally or by suppository. Hypertension can be treated with -blockers and encephalopathy with fluid restriction and electrolyte correction. Agitation should be treated with diazepam Valium ; or lorazepam Ativan ; , but it should be used with caution. Seizures are difficult to control medically. First correct hyponatremia. Antiseizure drugs have risks. The safer drugs are diazepam, magnesium sulfate, gabapentin Neurontin ; , and clonazepam Klonopin ; . Diazepam can be given IV. Cyclic, recurrent acute porphyric attacks are clearly related to female sex steroid changes in their blood, especially progesterone, that induce hepatic heme synthesis. One method of treatment is to administer recurrent intravenous heme, 1 to 2 days mid-cycle with each menstrual cycle. Another is to diminish hormone production by giving analogs of gonadotrophin-releasing hormone1 GnRH ; to prevent cyclic pituitary release of luteinizing hormone LH ; and follicle-stimulating hormone FSH ; . The GnRH analogues may be given orally or by an intranasal route. After 3 to 6 months, osteoporosis is a predictable serious problem. Osteodensitometry should be performed at 6-month intervals to monitor decline in bone mass. The decrease in bone density may be arrested by estrogen orally or as a transdermal patch and or alendronate sodium Fosamax ; . Specific Treatment. The basis for specific treatment is that there is a presumed hepatic heme.

Both my research and my many years as a clinician have convinced me that marijuana can serve at least two important roles in safe and effective pain management. Ample anecdotal evidence and clinical observations, as well as significant research findings, strongly indicate that marijuana, for whatever reason, is often effective in relieving pain. This is true across a range of patient populations, including the elderly, the terminally ill seeking comfort in their final days, young adults stricken with life-threatening conditions, and cancer patients unable to tolerate the devastating effects of potentially life-saving therapies. No experienced and respected physician will deny that for such patients opioid therapy is central to palliative care. By the same token, the same experienced physicians will readily acknowledge that opioids often induce nausea and vomiting. For a number of pain patients, standard prescription antiemetics e.g., Compazine, Zofran and Reglan ; simply do not substantially reduce their nausea. For many, those medications are substantially less effective, or produce more debilitating side effects, than marijuana. Quite simply, marijuana can serve much the same function for pain patients undergoing opiate therapy that it does for cancer patients undergoing chemotherapy: it suppresses the nausea and vomiting associated with treatment, and reduces the pain associated with prolonged.

These include reduced storm water runoff, increased air and water quality, storage and sequestration of atmospheric carbon and reduced energy consumption due to direct shading of buildings. This project is particularly important considering central Indiana includes two of the fastest growing counties in the nation Hendricks and Hamilton ; . Development and urban sprawl are happening at an alarming rate. We have less parks, green space, and tree canopy cover than most comparable major metropolitan areas in the country. Hoosier Heartland, a nonprofit organization of volunteers, will partner with the ten Soil and Water Conservation Districts in central Indiana, as well as other like-minded organizations and local businesses to make this project happen.
