Navitus started operating Jan. 1, 2004, in response to Rx payer demand for more transparency in the PBM industry DBN 3 12 04, p. 1 ; . Palmer contends the term "transparency" has since been corrupted by industry players. "Most of what transparency is in this industry today is not transparency, " he says. "It's shades of opaqueness. I know there is a lot of talk, a lot of rhetoric about transparency. I have yet to see any other PBM that approaches our level of pass-through." For example, according to Navitus, most traditional PBMs base claims transactions on a "per prescription" transaction fee. When utilization increases, so do PBM revenues, the company says. By contrast, Navitus uses a "per member per month" administration fee, under which increased drug utilization has no effect on revenue. The model has proven itself in Navitus' PBM contract with Wisconsin state employees, Palmer says. Navitus last fall renewed that contract, which covers 230, 000 state and local employees, retirees and their dependents DBN 11 3 06, p. 8 ; . Navitus has helped save Wisconsin $150 million over the past three years, Palmer says, adding that other states are starting to take notice. Palmer says that Navitus is eyeing more state business, but adds that no other contracts are imminent. "The states are one customer for this business that have really embraced a truly transparent -- according to our, for example, mefenamic acid dicyclomine. Nonetheless, P cells or their equivalent ; appear to be the critical link in all forms of AF. Without their presence it appears that AF cannot be triggered. In Sessions 35 and 37 of the CR and in the March issue of the AFIB Report the probable role of P cells in the initiation of AF was discussed. In addition to the evidence therein cited supporting that interpretation there has been additional research by the Cleveland Clinic February 2005 ; showing that the cells in the PVs that trigger AF react to pharmacologic agents in a fashion similar to the SA node P cells ; . : blackwell-synergy doi abs 10.1046 jce Two Taiwanese EP canine studies in 2002 revealed the presence of pacemaker type cells ?P cells ; in the SVC and PVs. : circ.ahajournals cgi content abstract 105 22 2679 : ijbem volume4 number2 115 PVI is effective at terminating LAF AF because it isolates triggering foci that are probably P cells. However, sometimes it is necessary to ablate foci other than the PVs. These other foci are located in sites such as the posterior left atrium, the right atrium, the coronary sinus, the SVC, crista terminalis and the ligament of Marshall. Sometimes AF triggering foci can even be located in the thoracic veins. As previously reported, P cells, normally only present in the SA and AV nodes, have recently been documented in the PVs of those with atrial fibrillation v. controls ; . Could all LAF initially manifest as brief episodes of tachycardia? PV tachycardia has only recently been described. Like AF, supraventricular tachycardia SVT ; in general is also increasing in frequency. Although PVI by an experienced operator is usually quite successful, left atrial scarring is a powerful independent indicator of PVI failure. : content.onlinejacc cgi content abstract 45 2 285 AF induced fibrosis probably enables abnormal circuits to reestablish contact with the left atrium post ablation. The fact that aldosterone RAS ; causes cardiac fibrosis and ACEIs and ARBs appear to be more effective in preventing recurrent and new onset AF in those with structural heart disease v. LAF ; further supports this interpretation. Philippe Coumel once wrote that we must always bear in mind the complexity of autonomic interactions and how cautious our AF hypotheses should be for they are simple compared to reality. Provocation. The evidence should have been suppressed. As a consequence, D.I.R.'s adjudication of delinquency was reversed. "Fair Share" and Collective Bargaining Agreements 1. Although the Indiana General Assembly amended I.C. 20-7.5-1-6 a ; in 1995 to remove the so-called "fair share" provision which required nonunion teachers to provide some financial support to the school employee organization see QR Jan.-Mar.: 97 ; , there are continuing disputes being reported. In Anderson v. Yorktown Classroom Teachers Assoc., 677 N.E.2d 540 Ind. App. 1997 ; , the court, in addressing a dispute occurring before the effective date of the statutory amendment, upheld the use of the American Arbitration Association AAA ; to arbitrate the issue of the amount "fair share" due the collective bargaining unit from nonmembers. The use of the AAA has been found to be an adequate independent, non-judicial means for addressing such issues. See, for example, Flosenzier v. John Glenn Ed. Assoc., 656 N.E.2d 864 Ind. App. 1995 ; , transfer denied 1996 ; . The court rejected the nonunion teacher's argument that AAA is inappropriate because the parties do not have input in the selection of the arbitrator, and that the rules for Alternative Dispute Resolution ADR ; should be applied. The court stated that ADR rules "apply only in cases that have been filed in the courts of this state." At 542. Because the selection of the arbitrator was not "the unrestricted choice of the union, " the procedure passes judicial muster. The use of AAA procedures in such matters was approved in Ping v. National Education Assoc., 870 F.2d 1369 7th Cir. 1989 ; . In contrast to Yorktown, supra, the Indiana Court of Appeals found two other contracts between school boards and bargaining units infringed unconstitutionally upon nonunion teachers' First Amendment rights to freedom of expression and freedom of association. In both Ford et al. v. Madison-Grant Teachers Assoc., 675 N.E.2d 734 Ind. App. 1997 ; , trans. den., and Anderson et al. v. East Allen Education Assoc., 683 N.E.2d 1355 Ind. App. 1997 ; , the contracts between the respective teacher unions and school boards established the "fair share" of nonunion teachers to be the same as for members. Unlike Yorktown there were no procedures to determine a "fair share" so as to "avoid the risk that their [nonunion teachers'] funds will be used.to finance ideological activities unrelated to collective bargaining." Ford, 675 N.E.2d at 738; Anderson, 683 N.E.2d at 1357. Both courts noted the fees were the same for union and nonunion members, but that full union dues include "non-chargeable union activities, such as political and ideological activities" to which the nonunion teachers may disagree. "Compelling nonunion teachers to subsidize political and ideological activities as a condition of employment violates the nonunion members' First Amendment right to freedom of expression and freedom of association." Ford, 675 N.E.2d at 738-39; Anderson 683 N.E.2d at 1357. Such a contract is "fundamentally unconstitutional." Anderson, 683 N.E.2d at 1357. The Anderson case is also notable because several of the nonunion teachers involved in this dispute are former or current members of the Indiana General Assembly. ; 26 and metoclopramide. ANOTHER PATH TO THE BOMB I enjoyed William Sweet's insightful, balanced and critical review of Critical As sembly and Stalin and the Bomb ["Two Paths to the Bomb, " March April]. His comparison of the American and Soviet nuclear-weapons efforts is particularly important in the light of the controversy surrounding the Hiroshima exhibit at the Smithsonian Institution. From my own studies of the German work on nuclear fission, I certainly concur with Mr. Sweet's analysis and would perhaps go even further: in all the wartime or immediate postwar nuclear-weapons projects, moral qualms came after the weapons had been built and used, not before. MARK WALKER Union College Schenectady, New York [Editor's Note: For a discussion of the German atomic bomb efforts during the Second World War, see Laurence A. Marschall's review of Michael J. Neufeld's new book, The Rocket and the Reich, on page 47.] LAST BIOLOGICAL FRONTIER? I was impressed with Nalini Nadkarni's discussion of the studies of the rain-forest canopy ["Good-bye, Tarzan, " January February], but I would dispute the statement attributed to Terry Erwin of the Smithsonian Institution ; , that the forest canopy is "the last biological frontier." The deep sea, the largest biotope on earth, is considerably less known than the canopy, and its biodynamics and animal life more enigmatic. Curiously, William Beebe, who provided Ms. Nadkarni with her epigraph, was even more of a pioneer in the deep oceans than he was in the jungles. In 1930 he descended in the first modern bathysphere, and he made many more dives to explore the depths and to investigate the vertebrate and invertebrate life of the abyss. RICHARD ELLIS New York, New York Nalini Nadkarni writes of the dramatic scientific increase in interest in the socalled last biotic frontier--the canopy of trees. Indeed, her own painstaking work on epiphytes has done as much as that of any other canopy scientist, and more, to establish canopy science as a major new field of scientific endeavor. One of the myths Ms. Nadkarni touches on is the belief that the canopy is home to 50 percent of the species in the biosphere. Although there is much evidence, my own included, to support the view that the canopy is home to many hundreds or even thousands of species of insects. I often feel like I in a daze. I have experienced vivid dreamlike scenes upon falling asleep or awakening. I have fallen asleep in social settings such as the movies or at a party. I have trouble at work because of sleepiness. I have dreams soon after falling asleep or during naps. I have "sleep attacks" during the day no matter how hard I try to stay awake. I have had episodes of feeling paralyzed during my sleep. I wake up at night with an acid sour taste in my mouth. I wake up night coughing or wheezing. I have frequent sore throats. During the night I suddenly wake up feeling like I choking. Other than when exercising, I experience muscle tension in my legs. I have noticed or others have commented ; that parts of my body jerk during sleep. I have been told that I kick at night. When trying to go to sleep, I experience an aching or crawling sensation in my legs. I experience leg pain or cramps at night. Sometimes I can't keep my legs still at night; I just have to move them to feel comfortable. Even though I slept during the night, I feel sleepy during the day and naprelan.

All of the dose-response relationships were started and completed at the same time. Rings were weighed wet and dry weight ; after some experiments. Wet weights were determined after blotting the rings on filter paper, and dry weights were obtained after air drying the rings at room temperature for 24 h. Histology Histological assessment of the endothelial cell layer was done on rings from control and heartworm-infected dogs. Morphological assessment was based on physical integrity of vascular wall components, aided by identification of an important constituent, the endothelial cell-associated antigen factor VIII 7 ; . These observations were made on tissues that had been fixed in either Formalin or buffered glutaraldehyde-picric acid, processed for sectioning by standard paraffinembedding methods and stained with factor VIII antibody. At least three sections from each area sample ; of vessel ring were examined. Drugs and Chemicals All drugs and solutions were prepared fresh daily. Methacholine, bradykinin, substance P, calcium ionophore A-23187, indomethacin, mefenamic acid, L-NMMA acetate salt ; , L-NAME, aminoguanidine, SOD, catalase, and norepinephrine bitartrate salt were obtained from Sigma Chemical St. Louis, MO nitroglycerin Nitrostat tablets ; from Parke Davis Morris Plains, NJ and methylene blue from Merck Sharp & Dohme Rahway, NJ ; . Mefeenamic acid and indomethacin were mixed 1: 4 by weight with sodium carbonate in distilled water. Drugs were added in 15- and 30-l volumes to 15-ml baths. Statistical Analysis Results are means 6 SE, with values of P , 0.05 taken as statistically significant. In all experiments, reported n is the number of dogs used. Relaxation responses are expressed as percent relaxation, with preconstricting level taken as 0% relaxation and complete relaxation as 100%. Constriction responses are expressed in grams. Between-group control vs. heartworm ; and within-group control 6 inhibitors or heartworm 6 inhibitors ; comparisons were performed by using a one-way analysis of variance and the least significant difference test. Weights were compared between groups. An injury reporting system has been developed to standardize the information collected on each athlete and to allow more accurate statistics to be compiled. The BC Games Society provides a standardized injury report form for reproduction. All injuries and treatments rendered at each venue must be recorded. The form should follow the patient from the time they are treated at a venue until they reach the Medical Clinic. Once treatment is finished, the form should be returned to the Medical Clinic for data entry filing. Past experience has shown that a control reception area in the Medical Clinic and a specific person i.e. Medical Office Assistant ; should be assigned to receive medical and venue reports and to generally control the documentation and reception area. This helps to increase the effectiveness of injury reporting throughout the Games.

Group of that have raised questions regarding their fate and effects in the enviPharmaceutical compounds are atheir fateemerging compoundsesperonment. Given their intended use, characteristics and, cially, their effect spectrum might differ considerably from those of "traditional" micropollutants. As stated by the authors, drugs have traditionally not been viewed as environmental pollutants. Nevertheless, their potential to cause a variety of physiological responses has raised concern for effects in the aquatic ; environment. Limited knowledge is available of the implications of exposure to complex mixtures of these compounds. Municipal sewage treatment plants STPs ; act as point sources of pharmaceuticals to the environment. It is known that low concentrations of pharmaceutical compounds, coupled with their metabolic characteristics, lead to their incomplete removal from most STPs. Some compounds have been shown to bind to sewage sludge and may thus enter the terrestrial biosphere when biosolids are applied as fertiliser and soil conditioner. It is therefore important to understand the fate and behaviour of pharmaceuticals during waste water treatment. In the study reviewed here, the aqueous concentration of five pharmaceuticals was determined after 0.5, 2.5, and 5 hours of exposure to either artificial sewage sludge or river sediment. Neither substrate was previously exposed to pharmaceuticals. The compounds tested log Koc values between brackets ; were: Ibuprofen 2.59 ; , paracetamol 1.79 ; , salbutamol 1.5 ; , propanolol HCl 3.3 ; , and mefenamic acid 2.66 ; . It was found that after rapid sorption, especially to the artificial sludge, water concentrations increased again. Only a low percentage of the pharmaceuticals remained sorbed after five hours of exposure, independent of the hydrophobicity of the compound. The results of this study indicate that processes other than hydrophobic partitioning play a dominant role in the partitioning of pharmaceuticals to solid phases. These processes might include ion exchange, surface adsorption to mineral constituents, complex formation with metal ions, and hydrogen bonding. Thus, sorption of human therapeutic agents may depend on factors such as pH and ionic strength. The authors conclude that a major research effort should be made to assess the significance of drug residues in terms of their persistence and potential environmental impact. This could help to provide a realistic risk assessment of these compounds as well as to help understand the health and environmental consequences of not removing them from the wastewater stream and receiving environment.
