The latter study did not report any case of NRT dependence in adolescent never smokers [27]. Similarly, studies in representative samples of the UK and Swedish general populations found no never-user of tobacco among users of NRT [28]. A review of postmarketing surveillance data in the USA found no report of primary dependence to the nicotine gum and patch, [29] and only 39 cases of dependence on the nicotine gum were reported per million prescriptions to smokers, in surveillance data [30]. Therefore, addiction to nicotine gum in never smokers is probably very rare. Furthermore, there may be few adverse consequences of being addicted to the nicotine gum, except for the financial cost and the inconvenience of permanent chewing. In particular, NRT products are safe even in patients with heart disease, [31, 32] and there was no untoward effect of 5 years of nicotine gum use in the Lung Health Study [33]. Thus, long-term use of NRT is not known to be harmful. Participants volunteered to take part in our study after finding a link to the questionnaire on Google. This enabled anyone who was looking for information on the nicotine gum to find the questionnaire easily. As a result, our sample consisted of a self-selected group of people who proactively searched the web for information on the nicotine gum, perhaps because they worried about their nicotine gum use. This sample is therefore not representative of all nicotine gum users. In particular, only 2% of USA households who purchased nicotine gums purchased them monthly for 6 months or more, [34] and only 6% of participants in a nicotine gum trial used the gum for 24 weeks or more [35] These figures contrast sharply with the median duration of 8 months of NRT use in ever smokers in our survey, which suggests that our method oversampled long-term users of NRT. It is therefore not possible to extrapolate from our data a population prevalence of never smokers among daily gum users. Abstract Blood nicotine and cotinine concentrations were measured in 27 volunteers before and after taking snuff. Within 10 minutes after snuffing blood nicotine concentrations were comparable to those obtained after the 10 minutes or so that it takes to smoke a cigarette. In daily and occasional snuffers increases in plasma nicotine concentrations averaged 77-7 and 12 3 nmol l 12 6 and 2 0 ng respectively, while the novices showed no appreciable increase. The increase shown by the daily snuffers was comparable to the average increase of 62 3 nmol l 101 ng ml ; obtained from a single cigarette by a group of heavy smokers. The peak nicotine concentrations in the daily snuffers were also similar to the peak values in 136 heavy smokers-222-6 and 226-3 nmol l 36 1 and 36-7 ng ml ; , respectively. Unusual multiple-dose snuffing produced massive increases in plasma nicotine to concentrations that have never been recorded in smokers. The similarity of the concentrations produced by regular daily snuffing and regular daily smoking suggests that the plasma nicotine concentration has some controlling influence over the self-regulation of these two quite different forms of tobacco use. The rapid absorption of nicotine from snuff confirms its potential as an acceptable and relatively harmless substitute for smoking.

Finally, a very brief update on what is happening with the EU chemicals policy. The EU strategy was published in February. The Environment Council ministers considered this in June 2001 and made a series of recommendations. These have been taken forward through a series of working groups, which started in October 2001 and will carry on working, probably for the next couple of months. The original timetable was that there would be proposals for new legislation in Spring 2002, but given that the working groups have not finished yet, that is probably a bit optimistic.
