Severe Acute Bleeding Severe acute uterine bleeding in the nonpregnant patient usually occurs in one of three settings: the adolescent with a coagulopathy most commonly von Willebrand disease35, 36 ; , the adult with submucous fibroids, or the adult taking anticoagulants. Initial management is based on hemodynamic stability as outlined in Figure 2. The patient is given high-dose estrogen orally or intravenously depending on bleeding severity ; and then a tapering schedule of oral contraceptives. One common oral contraceptive regimen is ethinyl estradiol 30 g norgestrel 0.3 mg eg, LoOvral ; 1 active pill 4 times daily for 4 days, followed by 3 times daily for 3 days, followed by 2 times daily for 2 days, followed by once daily for 3 weeks. The patient then stops the pill for 1 week and then cycles in the usual manner, 3 weeks on and 1 week off, for at least 3 months. Once the patient is clinically stable, an investigation into the cause of bleeding includes screening coagulation studies and possibly transvaginal ultrasound TVUS ; . The ultrasound may include a saline-infused sonohysterogram, especially when the endometrial stripe is thick, because of the increased sensitivity for endometrial polyps and submucous fibroids.37, 38 In general, ultrasound is less likely to be helpful at menarche, and instead the evaluation for coagulopathy, especially von Willebrand disease, becomes more relevant. Irregular Bleeding Irregular bleeding is a heterogenous category that includes metrorrhagia, menometrorrhagia, oligomenorrhea, prolonged bleeding that can last weeks or months, and other irregular patterns. These patterns were lumped together in the algorithm because their initial management is similar. Patients with minor variations of normal bleeding may not require the evaluation outlined in Figure 3. For example, irregular bleeding within 2 years of menarche is usually due to anovulation, secondary to an immature hypothalamic-pituitary-ovarian axis.21, 39, 40 However, adolescents may request more than simple reassurance and can be offered oral contraceptives or a progestin as described in the algorithm Figure 3 ; . Missed periods and prolonged intervals are expected in perimenopause.41, 42 Intervals may also decrease in the perimenopause, but repeated intervals less than 21 days or other irregular patterns require endometrial sampling. Intracellular targets for PGE2 actions are adenylyl cyclase, protein kinase A and glycogen phosphorylase. Concentrations of cAMP increased with increasing concentrations of PGE2, and peaked within 2 min of hormone application. In the presence of the phosphodiesterase inhibitor, isobutyl-3-methylxanthine, peak height was increased and peak duration extended. The protein kinase A inhibitor, Rp-cAMPS, counteracted the activation of glycogenolysis by PGE2, implying that the adenylyl cyclase protein kinase A pathway is the most important, if not exclusive, route of message transduction. PGE2 activated plasma membrane adenylyl cyclase and hepatocyte glycogen phosphorylase in a dose-dependent manner. The effects were specific for PGE2; smaller degrees of activation of glycogenolysis were noted for PGE1, 11-deoxy PGE1, 19-R-hydroxy-PGE2, and prostaglandins of the A, B and F -series. The selective EP2-receptor agonist, butaprost, was as effective as PGE2, suggesting that rockfish liver contains prostaglandin receptors pharmacologically related to the EP2 receptors of non-hepatic tissues of mammals. Emerging data indicate the superiority of higher mesalamine doses for moderate UC.15 However, a recent study indicates that most patients with moderate UC currently receive less than 4.8 g d mesalamine.16 In a retrospective chart-review analysis, community gastroenterologists from 39 practices abstracted data from their own practice charts of UC patients. Disease severity of each UC encounter flare was assessed by assigning a Physician's Global Assessment PGA ; of normal, mild, moderate, or severe.16 Of the 406 UC patients included in the analysis, 212 52.2% ; were classified as having moderate disease, 174 42.9% ; were classified as having mild UC, and 20 4.9% ; as having severe UC. Patients were also stratified into 3 groups according to induction mesalamine doses: low dose 2.4 g d ; , intermediate dose 3.2 to 4.0 g d ; , and high dose 4.8 g d ; .17 Analysis of physician prescribing patterns revealed that 72% of patients with moderate disease received less than 4.8 g d mesalamine as initial treatment Table 1 ; .16 These data suggest the existence of a clinical-care gap, as current treatment patterns do not reflect emerging data demonstrating the benefit of higher mesalamine doses as initial therapy in patients with moderate UC.
There has been considerable progress in the initiation of audit projects in the Forensic Division following the expansion of SHO numbers at Broadmoor Hospital. The new SHOs generally present as enthusiastic about getting involved in audit projects and to be generally well supervised. However, in this first cycle, the Directorates have been less successful in bringing projects to completion in a manner which would allow revisiting of these topics in subsequent audit cycles. At Ealing, there has been a better response over the last year, due in part to the introduction of the local audit supervision committee. The same issues regarding completion of audit projects in a way which allows for the reaudits remains. This will be a challenge for the following year. The Division intends to build on the model of the annual clinical risk review meetings, as part of the divisional clinical governance programme, with an annual audit presentation, where each Directorate will be asked to present the audit which has had the greatest impact on practice over the course of the last year as a divisional learning event.
