Glucocorticoid treatment produces rapid symptomatic improvements in critically ill patients, including temporary relief of fever, sweats, lethargy, weakness, nausea, and other nonspecific effects of cancer. Glucocorticoids also cause mild euphoria, a general feeling of well being, and a stimulation of appetite.155, 156 These effects are transient, and only short-term treatment is possible due to side effects of the high doses. Also, when glucocorticoids are withdrawn, adrenocortical insufficiency and patient discomfort can occur. For these reasons, corticosteroid treatment is normally reserved for patients whose life expectancy is brief a few weeks or less ; . While for most terminal patients opiods are the commonly employed option for pain manage ment, corticosteroid therapy remains a viable option, especially in cancers of the central ner vous system where edema is common. Abdominal visceral afferents: The role of the vagus nerve in triggering emetic response has been firmly established10. The AP is intimately related to another site named the nucleus tractus solitarius NTS ; which is the main site where the vagal fibres terminate. It is a co-ordinating site for the integration of visceral information as well as transmitting emetic signals from the visceral organs to the CNS. However some vagal afferents may terminate in the AP itself or be capable of activating it2, 8, 10. The vagal afferents involved in the emetic response are of two types a ; Mechanoreceptors which are located in the wall of the gut and are activated by both contraction and distension of the gut wall commonly associated with disruption of gastrointestinal motility. b ; Chemoreceptors that are located in the mucosa of the upper gut which respond to irritants like copper sulphate. It has been proposed that enterochromaffin cells are mainly activated by irritants which in turn activate vagal afferents. Vestibular system: Vestibular disturbances due to unusual motion are a common cause for emesis. The stimulation of the labyrinth structures in the inner ear causes motion sickness1. Impulses from the vestibular apparatus in the inner ear travel to the vestibular nucleus and then through the cerebellum to the vomiting centre. The transmission from the vestibular apparatus to the vomiting centre appears to be relayed through histaminergic and cholinergic fibres. The CTZ does not seem to have any role to play in this type of emesis15. However stress induced by excessive motion may play a role in the emetic stimuli16. Higher Centres: Emesis can also be induced by various stimuli like pain, unpleasant smell, taste and sights. Anticipatory emesis also sometimes occur in association with cytotoxic chemotherapy. The stimuli in these cases are presumably under the influence of higher centres. The higher brain stem and cortical structure are primarily implicated, but the exact mechanism is still under investigation8. 3. The main goal for this Program Performance Audit of Medical Care for Inmates is to provide an independent assurance and opinion about the economy, efficiency and effectiveness of those services. This Executive Summary is intended to present in brief CJI's activities during the performance audit of the Arlington County Detention Facility ACDF ; medical services. Following the description of the work performed is a brief description of our findings and recommendations. A thorough description of our findings and recommendations are profiled in Section III of this report. Prior to beginning the fieldwork, the project team requested and received numerous documents and reports compiled by Arlington County Sheriff's Office, other County agencies, public and private health care resources within Virginia, and relevant reports and documents prepared by or for other jails. In order to assess the performance of ACDF Medical Service delivery, the CJI team members have spent ten person-days 1 onsite during two separate site visits two weeks apart. Site visits were conducted during all three medical staff shifts2 in order to observe the medical services being delivered.
