At last, a book about money that tells it straight: put it under the bed. All of it. Sure, it makes for easy access to burglars but better them than the felons passing themselves off as financial advisors or acting as foot-soldiers for organisations with words like union, mutual, trust, alliance, equitable or assurance in their name. Mean With Money, inspired by Davies' well-loved column in The Sunday Times, is wilfully short on practical advice but offers instead good humour and much-needed empathy as we face the corporate horror of high-handed and indifferent financial institutions. Davies also looks at ingenious ways to save money cut your own hair, for starters ; and what to do with it when it arrives. Along the way, he reveals details of his regular visits to McDonald's it's free to use their toilets ; , the eccentric old ladies who staff his local Oxfam shop and the swim that cost him 333. When cocaine is snorted, the effects begin within a few minutes, peak in 15 to minutes and disappear after a few hours. Low doses produce a short period of euphoria and feelings of increased energy, alertness, self-esteem and sensory awareness. While artificially depleting the body's energy supply, cocaine reduces the perceived need for food and sleep and can cause impulsive behavior and mood changes. Smoking freebase produces a two- to fiveminute intense high. Because larger amounts get to the brain quickly, smoking also increases the risks of using the drug. Risks include confusion, anxiety, slurred speech and psychological problems. Smoking crack produces an intense and rapid euphoria commonly known as a flash high. Cocaine molecules reach the brain in less than 10 seconds. The resulting three- to five-minute high is followed by an unpleasant crash. The user feels irritable and agitated, and has an intense craving for more cocaine caused by the high concentration of the drug in the bloodstream. The initial high is never reached again, and subsequent lows get lower. This cycle reinforces the craving. Injecting cocaine produces an effect within 30 seconds that peaks in five minutes and lasts about a half hour. Users who inject run the risk of contracting hepatitis, AIDS, and other infections due to the sharing or use of contaminated needles.

| Individuals N 571 ; with incident cough plus phlegm are not included in this table, but their data for individual fruits can be found in the manuscript Table 3 ; . Models are adjusted for age, total energy intake, dialect group, gender, smoking status never, former, current ; , age at starting to smoke 20, 15-19, 14 years ; , and cigarettes per day 12, 13-22, 23 ; . These odds ratios are for individual fruit items without adjustment for the other items. Fruits were categorized into tertiles, based on the distribution of the entire baseline cohort. Median values within tertiles are presented in parentheses, and are based on the subset of the cohort used in these analyses. Tertiles rather than quartiles were used because consumption of any individual fruit is substantially less than total fruits. Tests for trend were conducted using the median value for each tertile of the specified dietary factor analyzed as an ordinal variable in the regression models.

Hospitals blame health care users for the crisis because employees want to be able to go to any hospital or physician they choose rather than being restricted to specific systems for the sake of volume discounts, for instance, gamma hydroxybutyrate.

Muted the debate? I read with inner discomfort the stories of pioneering professionals who had been fired for getting pregnant, alienated or subjected to great hostility. This was not my case. I had incredibly supportive family and supervisors who were nothing but empathic and positive about the situation. So why this nagging feeling that pregnancy was sapping away from my professional focus? The weeks rolled on to the second trimester the nausea lifted, the fatigue lessened. I felt my mental acuity return, my motivation and my drive. The baby growing inside me was making its presence known in a more pleasant way; quickening led to actual kicks; I got bigger; I became more protective over my growing child's safety, slowing down when driving and eating healthier. My previous resentment of how the pregnancy was affecting my professionalism gave way to fears of how being a doctor was going to affect my being a mother. I begrudgingly began to discuss my pregnancy with my longstanding therapy patients. We began to process how my upcoming leave and gravid state would effect them. My previous efforts not to selfdisclose in therapy, to maintain a neutral stance was now altered. The core elements of my life were on display for everyone to see. Intellectually I could grasp why it had to be discussed, as a pregnancy cuts.

Based on well-accepted criteria for identifying synaptic transmitters and neuromodulators in nervous tissues, the best evidence to date is that serotonin is a neuromodulator. That is, 5HT is released from serotonergic neurons embedded in the taste bud and regulates the properties of adjacent receptor cells. Certain side-effects of drugs, especially the tricyclic antidepressants that block monoamine uptake, may be attributable, in part, to serotonergic mechanisms in taste buds. Detailed examinations of the localization and characterization of 5HT receptors in taste buds; what triggers serotonin release from taste cells and how serotonin influences peripheral mechanisms of taste in intact animals, would greatly strengthen this conclusion.

Of extrapulmonary disease, as no HIV positives were identified. An alternative explanation for high rates of extrapulmonary disease in a population living in close contact with their cattle, and not boiling or pasteurizing the milk which forms their staple food, might be the impact of Mycobacterium bovis, historically associated with higher rates of extrapulmonary disease. Finally, because of the extremely limited access to TB diagnosis and treatment in South Sudan it is likely that only a small proportion of all TB cases are diagnosed. Care seeking beliefs and behaviour in the local population could substantially bias the types of TB patient presenting for care. Outcome and evaluation The observed default rate of 0.6% from the supervised 4-month intensive phase of therapy demonstrates that it is possible to treat patients successfully, with negligible risk of promoting drug resistance, under the difficult prevailing conditions. The high death rate among sputum-positive patients is probably due to treatment delay and very advanced disease at presentation. Seasonal tuberculosis treatment cohorts This highly unconventional approach involving seasonal provision of TB treatment was initially thought to be the only safe and feasible mechanism for providing treatment under the unique chronic conflict conditions prevailing in this area of South Sudan. Perhaps not surprisingly, this strategy proved fraught with problems. It caused intolerable professional ethical difficulties for staff who were expected to turn patients away after the closure of the recruitment period. It also led to tension between MSF expatriates and the community, at one point even fostering insecurity, when a chief and patient threatened the medical doctor. Moreover, since TB is not a seasonal disease and patients continue to present for treatment even when the admission period is closed, the community credibility of the programme, essential to the function of passive case finding, is seriously compromised. Because of these difficulties, the programme was opened to year-round admission about 6 months after closure of the initial cohort. This experience reinforces the position that year-round treatment remains the default position of any TB programme. Conclusions TB in complex emergencies and conflict settings is an underrecognized priority. New ideas and innovative strategies applying established principles of TB treatment to unique and difficult situations are needed to give people living in settings such as Sudan access to this lifesaving care. Based on the experience of this pilot project, 2 more sites for TB treatment have been opened in Upper Nile and the Nuba mountains. One expected outcome of a TB programme--reducing TB transmission in the community--requires the detection of 70% of all patients. This may not be achievable under conditions of conflict and insecurity.
