This is especially so as sponsors get more comfortable with handing over an NCE to a CRO at a very early stage for purposes of developing the manufacturing process and or formulation. If the CRO participated in setting specifications or didn't perform a labeling function according to FDA regulations, it may be liable for a product defect and incur product liability, Mackler points out. In such situations, the CRO may be deemed to have strict liability, meaning it is responsible if an adverse event occurs as result of something it knew or should have known. According to Mackler, CROs face two other types of product liability exposure: failure to follow FDA or other government requirements and failure to perform according to GMP or GCP standards. For instance, if a CRO or CMO fails to follow GMP standards and a defect in the drug results, then there's liability both to the CRO and the sponsor. "What we've been finding out is the sponsor relies on the CMO to fulfill certain duties -- but that CMOs, really, many times, don't always act in the full interest of the sponsor relative to the nature of the contracts, " Mackler adds. Mackler's advice to sponsors: Whether they use a CRO or CMO, they should still take the responsibility for making sure that laws, regulations and FDA requirements are being fully met. Abstract Body: Background: Nodal metastasis is recognized as a powerful prognostic marker in breast carcinoma, but conflicting reports exist for the precise involvement of VEGF-C and VEGF-D in lymphatic invasion and lymph node metastasis. The aim of this study was to determine whether serum VEGF, VEGF-C, VEGF-D levels and VEGF-C D ratios correlate with lymphatic invasion, lymph node metastasis, CA 15-3, CRP and IL-6 levels in patients with breast carcinoma. Methods: In this prospective study, the serum levels of VEGF, VEGF-C, VEGF-D, CA 15-3, CRP and IL-6 are determined by ELISA in patients with pretreated breast carcinoma n 80 ; and healthy women n 20 ; . The volume of the tumor was calculated using threedimensional tumor sizing. Comparison of data between patients and healthy controls; and relationships between circulating factors and clinicopathological variables were analyzed using the Mann-Whitney U or Kruskal-Wallis tests. Results: The mean age of the patients was 50.7 13.5 years. The mean level of VEGF-C was significantly higher in patients than control p 0.033 however, no statistically significant difference was observed for either serum VEGF and VEGF-D levels. Increased VEGF-C levels were detected in patients having smaller volume and low nuclear-grade tumors p 0.053, p 0.061, respectively ; . INTRODUCTION Cystic echinococcosis CE ; is a serious health problem in endemic countries including Turkey and many other Mediterranean countries 1-3 ; . It is caused by Echinococcus granulosus, which enters portal circulation from the upper intestine and lodges in the liver, the lung, and other organs and tissues where it develops into cysts. Although surgery is still widely practiced for most patients with CE, medical therapy with benzimidazoles mebendazole or albendazole ; either alone or in conjunction with surgery has been reported to be effective in a significant percentage of patients 4-7 ; . However, the safety and efficacy of mebendazole in young children with CE is limited 8, 9 ; . We herein report a pediatric case of a rare form of disseminated CE affecting the lung, the liver, and the kidney that was successfully managed with mebendazole without side effects. PATIENT AND LABORATORY INFORMATION A 4-year-old girl presented with fever, cough, abdominal pain, and weight loss of 2 months duration. She was living on a farm and had close contact with dogs and sheep. Physical examination showed malnutrition and a body weight in the less than the 25th percentile, diminished breath sounds, and massive hepatomegaly of 10 cm palpable below the right costal margin. Laboratory investigations including complete blood count, urine analysis, and blood chemistry gave normal results excepting 8% eosinophilia on white blood cell differential. Anterior-posterior chest x-ray showed shadows of numerous cysts of similar size 2 - 3 cm ; filling both lung fields Figure 1A ; . An abdominal ultrasound examination revealed widespread cystic lesions ranging in diameter from 1 mm to both lobes of the liver and one large cyst 3 cm in diameter in the left kidney Figure 2A ; . Indirect hemaglutination test for cyst hydatid was positive in 1 3, 200 titer 1, 10.
