Within Europe, there appears to be regional differences in incidence of disease as well as differences in life span and degree of independence in later years. The gut microflora is amenable to modulation using probiotics, prebiotics and synbiotics, and the elderly represent a population group for which such strategies may be particularly suitable. CROWNALIFE has confirmed that the gut microflora within elderly people differs from younger adults, showing a much greater degree of species diversity. Through generation of 16S rRNA based phylogenetic inventories of the faecal microflora of healthy elderly individuals 65 years ; the elderly 80% of cloned 16S rDNA sequences corresponded to previously uncultured bacteria while 1 3 of these represented novel phylogenetic lineages. In a pilot study examining microflora composition within different age groups in two different countries, the increased microflora diversity has been confirmed quantitatively using a set of 16 phylogenetic probes. Ursodeoxycholic acid UDCA ; has been considered the main treatment to PBC patients through a decrease in the detergent effect of endogenous bile acids, concomitantly reducing the hemolytic process. We report the case of a female patient with AIHA complicating PBC, treated with short course prednisone and UDCA, with good response. It makes a kind of midrashic sense but it does not form logically-coherent narratives, since, as Robbins points out, "If no major elements of the story are suppressed, repressed, dissociated, or discarded in a reductionistic manner, coherence and clarity may be attained at the cost of psychic and objective reality." 21 According to Elias Lipiner, in his review of the events leading up to the mass forced conversion of Jews in Portugal in 1497, on the order of King Dom Joo these approximately two thousand young children "were seized and sent to settle the inhospitable island of So Tom, where they were educated in the Catholic religion. According to the chroniclers, many of these children were devoured by wild animals." 22 In his notes, Lipiner cites for these two assertions--the education of the Jewish children and their being devoured by wild animals--one Portuguese, Rui de Pina's Chronica d'elrey D. Joo II, and two Jewish sources, Immanuel Aboab's Nomologia o Discursos Legales Amsterdam 1629 ; and Samuel Usque's Consolao s tribulaes de Israel Ferrara 1553 ; .23 On the face of it, there are two different kinds of historical discourses here, one, the official Christian chronicler making an attempt to rationalize the abusive treatment of the Jewish children by claiming they were handled as such minor converts normally were in Portugal, i.e., fostered to families or monasteries for religious instruction and upbringing. However, there is no documentary evidence to back up such positive treatment of these child slaves sold to the Portuguese captains who were attempting to establish So Tom as a viable economic venture.24 On the other, the face less easy to pinpoint in a normal historical approach, there is a mythic account of the terrible loss to the Jewish community of so many children to the brutality and cruelty of kidnapping, isolation, and exploitation, the devouring by wild animals standing metonymically for other forms of death, such as disease, overwork, and beatings by task-masters. These midrashic sources--poetic, mythic, allegorical, etc.--focus on the most dramatic of images of what actually happened to these young victims. These dramatic images serve as triggers to emotional states by releasing suppressed memories, somatic tensions in the very muscles, as it were, of the bodies in question. These are chemical hormonal ; changes in the physiology of the persons involved and can be traced in the bizarre behaviours and attitudes they display, although, of course, we can neither measure them with scientific accuracy nor interpret private and confessional statements. They are also midrashic triggers that pull together disparate image-complexes, break open words and phrases locked into conventional iconographic models, and reconfigure new imagery by radical reading-in.25 What is dredged up and put together by these individRobbins, "Oedipus" p. 23. Pression Fig. 4A and B ; . Expression of spliced TNF- mRNA, by contrast, was strongly inhibited at 24 h and beyond Fig. 4C ; . In the presence of 2-AP levels of precursor transcripts had increased by 7.6-fold at 24 h, relative to that of the untreated control Fig. 4E ; , while levels of mRNA had decreased by 7.7-fold Fig. 4F ; . These representative kinetic data demonstrate a significant decline in the flow of precursors into TNFmRNA. They show that the accumulation of precursor transcripts occurs promptly and in the same cells in which the accumulation of TNF- mRNA is inhibited. In agreement with an earlier report 32 ; , expression of mRNA from a cotransfected CAT gene was not noticeably affected by 2-AP Fig. 4D and G ; , showing that efficiency of transfection was constant. The 2-AP-sensitive mechanism that supports splicing of TNFprecursor RNA in PBMC thus is also present in BHK-21 cells. 2-AP elicits accumulation of unstable, functional TNFprecursor transcripts. The accumulation of unspliced TNFprecursor transcripts occurred when 2-AP was present from the outset of induction Fig. 2A ; . In the experiment shown in Fig. 5, 2-AP was introduced later, at 3 h, a time at which precursor levels were still rising. Within 1 h, an extensive increase in the level of precursor transcripts was observed. Their level became maximal within 3 h before declining gradually, with large amounts of precursors still present 4 h later Fig. 5 ; . However, this increase in the level of precursor transcripts was not matched by a decrease in the level of TNF- mRNA. Indeed, when added after 3 h of induction, 2-AP failed to affect the subsequent expression of mRNA perceptibly, including the rise between 4 and 6 h, showing that splicing was no longer blocked by that time and that the presence of 2-AP did not affect TNF- mRNA stability. Because mRNA could still be generated, the 2-AP-induced rise in the precursor transcript level apparently involves functional TNF- RNA molecules.

RESULTS Surgical reduction of the blood flow to the anterior left ventricular wall lead to slight discoloration of the affected myocardium Fig. 1A ; and a decrease of the oxygen tension in the affected region from 46 4 to mmHg after 15 min Fig. 1B ; . To see whether the reduced oxygen tension resulted in stimulation of HIF-1 regulated gene transcription, we quantified VEGF mRNA. After 2.2 0.2 h, the expression of VEGF mRNA was increased 1.8-fold in the hypoxic region compared with normoxic myocardium P 0.001, Fig. 1C ; . The content of BNP mRNA in the hypoxic myocardium was increased 3.5-fold compared with that in normoxic myocardium P 0.005, Fig. 2A ; . A positive correlation was found between the increase in VEGF and BNP mRNA contents in the hypoxic myocardium r 0.66, P 0.05, data not shown ; . The porcine myocardium contains significant amounts of immature BNP pre-mRNA 37 ; . Quantification with a BNP pre-mRNA specific real-time PCR assay revealed that the BNP premRNA content was 4.8-fold higher in the hypoxic compared with the normoxic myocardium P 0.005, Fig. 2B ; . The VEGF mRNA, BNP mRNA, and BNP pre-mRNA contents were similar in the normoxic anterior and inferior left ventricular walls in control pigs Figs. 1C, 2A, B ; . To assess whether the increased BNP gene expression in hypoxic ventricular myocardium was accompanied by increased BNP peptide production, we analyzed BNP and proBNP peptides in myocardial tissue with radioimmunoassays and immunohistochemistry. However, neither hypoxic nor normoxic left ventricular myocardium contained detectable proBNP concentrations by RIA Fig. 3A ; or on immunohistochemistry Fig. 3B ; . In contrast, left atrial tissue contained both BNP and proBNP peptides, with a markedly lower BNP than proBNP concentration Fig. 3C ; . Confocal laser-scanning microscopy of immunostained atrial sections showed a granularlike appearance of proBNP extending along the contractile apparatus in the myocytes Fig. 3D ; . Gel chromatography of medium from freshly harvested atrial cells kept revealed two forms of proBNP-derived peptides, i.e., the intact precursor and a smaller N-terminal fragment Fig. 3E ; , and a small amount of BNP. The plasma proBNP concentrations in hypoxic pigs increased 2 h after induction of myocardial hypoxia P 0.028, Fig. 4A ; . In control pigs without surgical reduction of the blood flow, the proBNP plasma concentration was not significantly different from baseline values 9510.2%, P 0.62 ; . The absence of BNP and proBNP peptides in the ventricular myocardium may reflect rapid release of newly synthesized peptide from the myocytes. To examine this possibility, we kept freshly harvested ventricular cells for 3 h in serum-free culture medium. Although the cells did not contain detectable amounts of BNP or proBNP peptides before or after incubation, we found proBNP accumulation in the culture medium P 0.0001, Fig. 4B ; . The oxygen tension and pH did not change in the culture medium during the experiments data not shown.
