The objectives of this study were: 1 ; to determine the effectiveness of a telehealth intervention on knowledge and self-management of heart failure, and 2 ; to test whether the intervention improves clinical outcomes. We hypothesized that the use of telehealth facilitates self-management of heart failure, with subsequent effects on clinical status. We compared the effects of two different types of telehealth one-way monitoring vs. interactive video ; on the health care delivery process and patient outcomes. The sample consisted of 290 patients from 10 home health agencies. Eligible, consenting, patients were randomly assigned to either the intervention or control group and followed for 120 days. Data were collected by a blinded research assistant via telephone interviews on admission, 60 days and 120 days after admission. The Omaha System Problem Rating Scale for Outcomes PRSO ; was used to assess patient knowledge and behaviors; clinical status was measured with 4 items from the PRSO instrument. The Self-Care of Heart Failure SCHF ; instrument was used to measure self-management behaviors. Using a General Linear Model GLM ; across all three time periods, with clinical status as the dependent variable, we found that telehealth patients improved more than control group patients. Further post-hoc analysis found that patients in the monitoring sub-group improved more than patients in the video group. This difference was 5.236, p .01 ; . There were no significant differences in statistically significant F knowledge or self-management between groups. Patients in the telehealth group had fewer hospitalizations and fewer ER visits than patients in the control group. Patients who used the monitoring systems had the lowest hospitalization and ER visit rates, compared to patients in the video group and the control group. These differences approached statistical significance. Our findings suggest that daily monitoring may have the greatest impact on quality of care and clinical outcomes.
